Online Enrollment Forms

IN ADDITION TO THE ENROLLMENT FORMS, PARENTS OR GUARDIANS OF NEW STUDENTS MUST PROVIDE THE FOLLOWING:

1. BIRTH VERIFICATION

Parents/guardians of children new to the district will be asked to provide an official copy of the birth certificate for school files.

2. IMMUNIZATION RECORDS

Requirements are listed on the Oklahoma State Department of Health’s website. Contact your regular doctor or the OK State Department of Health for locations to obtain immunizations your child may still need.

Tdap Immunization Requirements for ALL 7^th Graders.

3. PROOF OF RESIDENCY

May include the following, but not limited to:

Proof of payment of local personal income or property taxes
Title to residential property in the district, or a valid unexpired lease agreement, or current receipts for payment of rent on a district residence where the student lives
Proof of current utilities (gas, water, or electric bills)

4. CDIB Card (If you are a registered Native American)

Transfer Requests

Transfers are approved or denied based on student’s standing (attendance, behavior), enrollment numbers, and space available at the site requested. For the 2020-2021 school year, decisions on new transfer requests will be made when enrollment confirmation is complete. Decision notification will be before the first day of school.

General Information on Transfer Requests

Before requesting a transfer, parents should understand they are responsible for providing transportation for their child to and from school, and the transfer may be revoked by the school principal if the student’s grades, attendance (absent and/or tardy), transportation arrangements, or behavior reach an unacceptable level.

Transfer requests must be renewed annually.

All transfer requests must contact the central office at (580)668-2306.

Parents, please complete each of the following forms for your student that is attending Wilson Public Schools next year:

NEW STUDENT ENROLLMENT
New Student – Student that did not attend Wilson last year or that left and did not finish the school year at Wilson.
New Students MUST complete ALL enrollment forms listed below and and bring documentation to the office of the building in which they are enrolling.
Documentation: 1. Birth Certificate, 2. Immunization Records, 3. Proof of Residency, & 4. CDIB Card (if applicable)

CURRENT STUDENT ENROLLMENT
Current Student – Student that attended and finished the school year at Wilson last year.
Current Students are required to complete all enrollment forms each year, with the exception of the Home Language Survey and Title VII Indian Verification Form. There is no need to complete those two forms if you have completed them at Wilson in the past. If you are unsure if you completed the two mentioned forms in the past, go ahead and fill them out again this year.
